The Provincial Grand Master takes a great interest in the reasons why a Brother has been Excluded or suffered Cessation. It would be very much appreciated if you would take a short time to fill in this form so that he, and other members of the Provincial team, may be made aware of the reasons.

Please ensure that you chose the correct category by reading the notes below:-
Cessation (Rule 148) occurs automatically after non payment of lodge dues for two years. The date recorded is the last day of the Lodge�s financial year when this condition is met.
Exclusion (Rule 181) can only apply for non payment of Lodge dues as specified in your Lodge bylaws and apply from the date of the ballot at a regular Lodge meeting.
